If you need to be listed as a certificate holder or additionally insured, please fill out the form below to request the certificate from our insurance agent. There will be a $100 charge for additionally insured status, and you should receive it via email within 1-2 business days.

***Insurance is renewed annually. If your event will take place after the expiration date listed on the COI, please return to download your document or make your request for additionally insured status after the insurance renews.***
